What goes in the visit request?
Your name, a phone number where you can be reached and the county you are in are enough. A short note on the kind of crash and the date it happened helps me prepare for our call. If you are filling out the form for an injured family member, say so in the note.

What does the form not do?
Submitting the form does not create an attorney-client relationship. Please do not send confidential details through it. Save those for the phone or video call, where we can talk them through. The website disclaimer explains this in more detail.
